How do I get in contact with Kelly Bazan at Clearview Counseling to make an appointment?

If you would like to connect with Clearview Counseling, please leave a message via the secure client portal.  You are also welcomed to send an email or voicemail; however, these two methods are not HIPAA compliant.  Please allow at least one business day for response to any messages. 

In the event of an emergency, please contact your medical doctor, call 911, and pursue 24-hour assistance from a local emergency room or police department. Clearview is not an emergency response clinic.

What if I cannot attend my scheduled appointment?

Your appointment time has been reserved for you.  Please give 24-hour notice to reschedule or cancel so that others may have the opportunity to be seen.  A fee of $130.00 will be automatically assessed if this policy isn’t honored.  You are welcome to email, text or connect on the client portal.  (Insurance does not cover the late cancellation/no show fee.)

How do I know if my insurance will cover mental health therapy? 

It is your responsibility to confirm coverage and know your co-pay or deductible (amount not covered by health insurance). If services rendered are not covered by your health insurance company, you are fully responsible for all services rendered. Please contact your health insurance company prior to your appointment to verify your outpatient behavioral health benefits and secure any pre-authorization requirements. If a required authorization is not obtained, you will be responsible for payment of services. Please notify my office immediately of any changes in your health insurance information or coverage. 

Will my information be kept private?

The information you share during session is held in strictest confidence and may not be released to anyone without your written consent, as prescribed by law. There are a few exceptions to this, which are regulated by state law, in which your therapist is required to report this information to the proper authorities (i.e. in cases of suspected child, elder, or dependent abuse or if a person poses a serious and imminent danger to oneself or someone else). If you would like your therapist to contact or share your Protected Health Information with another person or agency, you will need to complete a Release of Information form that will be kept on file. Only information pertaining to health insurance and billing will be released and will be held in upmost confidence. Your signature below authorizes the Release of Information necessary to process claims (Section 12 of the HFCA 1500 forms) and the payment of medical benefits and office fees for services rendered, for the purpose of insurance filing and client billing.

How will I be charged for sessions?

By signing this credit card authorization agreement, you authorize Clearview Counseling Center, LLC to make charges to your Health Savings Account, flex, debit/credit, or other card on file for sessions (including co-pays, deductibles, and/or coinsurance and insurance denials of claims), late cancellation/no-show fees, letter-writing, phone consultation fees and other office fees. The card(s) will be processed when there is a balance due.
